Beginning in July, 2010, all employees, including full time, part time, and substitutes may elect to receive pay by one of two methods: 1) Direct Deposit; or 2) Debit card (Depository bank account with a debit card). All pay will be electronically sent to either the employee’s bank via direct deposit or added to the employees debit card account.
The Direct Deposit Application form is to have your paycheck automatically deposited to a banking institution of your choice. Upon completion and receipt of your form, your direct deposit will be initiated on the pay period which follows. Please attach a "Voided" check to the application and return it to the Payroll office.
***You can only have one direct deposit and/or one Optional deduction. You can sign up to have an optional deduction taken from your pay check to be applied to another account. "See Optional Deduction Form"
The employer must make change in withholding no later than the first payroll period ending on or after the 30th day from the date the employer receives the amended Form W-4. It remains in effect until a new form is provided or until the IRS tells the employer to withhold on a different basis.
Invalid Form W-4 (IRS-Publication 15)
Any unauthorized change or addition to Form W-4 makes it invalid. This includes taking out any language by which the employees certifies the form is correct. A Form W-4 is also invalid if, by the date an employee gives it to you, he or she indicates in any way it is false. An employee who submits a false Form W-4 may be subject to a $500 penalty. A Form W-4 is invalid, withholding will default to single and zero.

Exemption from Federal Income Tax Withholding. (Exempt)
Generally, an employee may claim exemption from federal income tax withholding because he or she had no income tax liability last year and expects none this year. See the Form W-4 instructions for more information. How-ever, the wages are still subject to social security and Medicare taxes. See also Invalid Forms W-4, later in this section.
A Form W-4 claiming exemption from withholding is effective when it is filed with the employer and only for that calendar year. To continue to be exempt from withholding in the next calendar year, an employee must give Aldine Payroll department a new Form W-4 by February 15. If the employee does not give Aldine Payroll Department a new Form W-4 by February 15, begin withholding based on the last Form W-4 for the employee that did not claim an exemption from withholding or, if one was not furnished, then withhold tax as if he or she is single with zero withholding allowances. If the employee provides a new Form W-4 claiming exemption from withholding on February 16 or later, Aldine Payroll may apply it to future wages but do not refund any taxes already withheld.
